Deliver high-quality, patient-centered care in the home setting
Maintain primary responsibility for assigned caseloads, including assessment, care planning, implementation, and evaluation
Conduct initial and ongoing nursing assessments addressing physical, psychosocial, and environmental needs
Make regular patient visits and re-evaluate care needs throughout the episode
Communicate significant findings and changes in patient condition to physicians, supervisors, and care team members
Update and revise plans of care as patient needs evolve
Ensure all documentation meets clinical, regulatory, and HIPAA standards
Collaborate closely with interdisciplinary team members to support optimal patient outcomes
Current, unrestricted RN license (state of practice)
1+ year of clinical nursing experience
Willingness to travel within the branch coverage area (approximately 50%)
Valid driver’s license, dependable vehicle, and current auto insurance
Associate or Bachelor’s Degree in Nursing (preferred)
Home Health or Hospice experience (preferred)
Experience with
Home Care Home Base (preferred)
Competitive salary: $90,000–$115,000 annually
