About the role

We’re looking for a Lead Practitioner (Band
6)
to join our West Neighbourhood Mental Health Team (NMHT), based at Mill View Hospital.

This is a great opportunity to develop your community mental health skills within a supportive, friendly and well‑established multidisciplinary team
, working with people who have a wide range of mental health needs.

You’ll be part of a team that genuinely values staff wellbeing, learning and connection
, with a strong emphasis on collaboration and reflective practice.

What you’ll be doing
  • Completing clinical assessments, including risk assessment and formulation
  • Supporting service users with severe and enduring mental health difficulties
  • Working closely with carers, families and partner organisations
  • Delivering a range of evidence‑based clinical interventions
  • Building strong links with third‑sector organisations and specialist services
What we’re looking for
  • Registered Mental Health Nurse, Occupational Therapist or Social Worker
  • Experience working with people with complex mental health needs
  • Confidence in assessment, risk management and care planning
  • A collaborative, motivated and compassionate approach

Previous community experience is desirable but not essential.
