Overview
Case Manager-Connecticut Collaborative Reentry Housing. Catholic Charities of Fairfield County is seeking a Case Manager for its CCR Connecticut Collaborative Reentry Housing program located in Bridgeport, CT. This full-time (35-hours/week), benefits-eligible position is responsible to implement Catholic Charities’ strategic mission through the core function of strengths-based client support which will include but not be limited to: assessment, goal planning, service referral and coordination, training, and advocacy.
Accountabilities- Collaborate with Housing Department team and agency staff in providing client services by maintaining quality assurance standards in assigned housing program.
- Conduct initial and periodic assessments: work with clients to develop individualized strengths-based services plans that reflect attainable and measurable goals.
- Link clients to community resources based on identified needs and service plan objectives. Services may include but are not limited to: income, entitlements, vocational planning, life skills, treatment, child care, and legal assistance.
- Work in collaboration with landlords and property managers to serve as an advocate and point of contact for client housing stability.
- In accordance with funding and agency compliance standards, complete and properly maintain clear, concise, and accurate records of client services including but not limited to: assessments, progress notes, service plans, charts, incident reports, log books, and all other documentation related to client activities.
- Monitor client behavior, program activities, and provide support for the safety of all clients.
- In accordance with funding and agency compliance standards, properly complete and submit all program quality assurance reports as related to client services and demographic information.
- Participate in all required CCFC Housing Programs trainings, meetings, workshops, and activities related to staff professional development and community collaboration.
- Assist clients with preparing for housing, including affordable housing search, transition to housing, tenancy skills building, fiscal preparedness for independent rent payment, and housing maintenance.
- Assist clients in obtaining additional in-home services, when appropriate, including visiting nurse, mental health waiver services, and senior care services.
- Assist clients with identifying unaddressed needs related to benefits, income, medical conditions, mental health, substance use and recovery, family and/or community-based supports, and access to mainstream services.
- Work collaboratively and maintain positive, professional communication with property owners/managers, and community providers to support housing acquisition, retention, and overall stability.
- Create strength-based person-centered service plans in collaboration with the client.
- Actively participate in agency and community-based meetings and trainings to engage in programmatic, operational, and client service best practices.
- Maintain timely and accurate documentation for all client files, including assessments, service plans, client services, referral outcomes, housing inspections, rental assistance, and case notes. Comply with agency, funder, and statewide HUD HMIS standards.
- Associates or Bachelor’s Degree in Human Services or related field or High School Diploma/GED and relevant work experience required.
- Experience In Data Management And Reporting Required.
- Trained in HMIS and DDaP systems preferred.
- Licensed HMIS user preferred.
- In accordance with funding contracts, actively participate as part of a cohesive team within CCFC Housing Programs in order to provide quality client service.
- Must demonstrate advanced communication and problem solving skills.
- Must demonstrate advanced computer skills.
- Knowledge of supportive housing policies/procedures and federal regulations preferred.
- Valid driver’s license, vehicle, proof of current registration and insurance required.
