Optimus Health Care, the largest provider of primary health care services in Fairfield County, is seeking an Immediate Care Clinician to join our team at the Park City Primary Care Clinic in Bridgeport. Position Summary

The Immediate Care Clinician will provide primary care to patients following accepted standards and protocols, functioning as part of a team. Clinical care will be delivered on a scheduled and walk‑in basis, with clinical guidance and direction provided in collaboration with the Chief Medical Officer.

Essential Functions & Responsibilities
  • Provide direct patient care, including pre‑operative care, post‑hospital follow‑ups, up to four physical exams per day (30 minutes each), urgent care, and telehealth services as needed.
  • Collaborate with the CMO to provide clinical guidance to non‑physician team members regarding direct patient care, use of clinician time for in‑person and telehealth visits, and coverage questions.
  • Provide clinical coverage at various Optimus sites as required while maintaining a primary presence at a designated clinical location.
  • Monitor the Immediate Care pool daily, distributing assignments based on need and clinician staffing availability.
  • Follow up on own patients’ lab and imaging results (outside of PTO, responsibilities will be assigned to another team member).
  • Support primary care clinicians in completing home care, other clinical orders, and documentation as needed.
  • Participate in scheduled Immediate Care team meetings to maximize care process efficiency and design standing orders to guide patient care and reduce administrative workload.
Job Qualifications/Requirements
  • Strong interpersonal communication skills with the ability to work effectively with a diverse patient population.
  • Ability to work independently and as part of a team, interacting with colleagues and other staff.
  • Knowledge of clinical operations and procedures.
  • Comprehensive knowledge of modern general medicine and public health principles.
  • Ability to work under pressure, analyze and evaluate individual needs, reach sound conclusions, and make appropriate recommendations.
  • Detail oriented and able to multi‑task effectively.
  • Knowledge of legal and ethical standards for medical care delivery.
  • Ability to maintain quality, safety, and infection control standards.
  • Knowledge of current principles, methods, and procedures for medical evaluation, diagnosis, and treatment in the area of expertise.
  • Knowledge of community medical diagnostic and patient care services.
  • Ability to supervise, advise, and train clinical professionals.
  • Knowledge of relevant drugs and patient care aids, with ability to prescribe dosages and instruct patients.
  • Ability to develop and present educational programs and workshops.
  • Competent with common PC applications (Internet, Email, Microsoft Office) and comfortable with EMR applications (EPIC).
  • Knowledge of accreditation and certification requirements and standards.
  • Effective verbal and written communication skills.
  • Experience with telemedicine is helpful.
Education

Must hold MD, DO, or graduate of an AMA‑accredited Physician Assistant or NP program. BA/BS or MS preferred.

Experience

2 years of experience preferred in patient care as MD, DO, PA, or NP. Board certification in internal medicine, family practice, or emergency medicine preferred.

Language Skills

Bi‑lingual preferred.

Licensure / Certification
  • Current unrestricted Connecticut license to practice medicine (MD, DO, PA, NP).
  • Active Federal DEA and Controlled Substance Registration.
  • For physicians, board certification in a specified area of medical specialty.
  • Current CPR and ACLS certification required.
