- Conduct comprehensive health assessments, including obtaining medical histories, performing physical examinations, and ordering/interpreting diagnostic tests.
- Develop and implement individualized treatment plans in collaboration with the supervising physician, focusing on health promotion, disease prevention, and management of acute and chronic conditions.
- Prescribe medications, order therapies, and manage medical devices according to state regulations and collaborative practice agreements.
- Provide patient education and counseling regarding health maintenance, disease management, medication adherence, and lifestyle modifications.
- Evaluate and manage common acute illnesses and chronic conditions (e.g., hypertension, diabetes, asthma).
- Perform minor office procedures as trained and within scope of practice.
- Collaborate with physicians, nurses, medical assistants, and other members of the healthcare team to ensure coordinated patient care.
- Document all patient encounters accurately in the electronic health record.
- Participate in quality improvement initiatives and adhere to organizational policies and procedures.
- Serve as a resource for medical staff and participate in educational activities as needed.
- Provide age-appropriate care to patients across the lifespan.
- Manage referrals to specialists and coordinate follow-up care.
- Maintain current knowledge of family medicine best practices and engage in ongoing professional development.
- 1-2 years clinical experience in a primary care/internal medicine setting.
- Active PA or NP in family medicine specialty certification.
- Active, unrestricted CT medical license or in process of obtaining.
- ACLS/BLS/PALS (as required) Certification
- DEA License
40 scheduled hours, M-F, 7am-5pm
Skill SetPrefer 1-2 years of clinical experience as a NP/PA within the stated specialty/service line. APRNs, PAs, CRNAs, and CNMs are required to be appointed to the Affiliated Medical Staff of a YNHHS Hospital and credentialed through the Medical Staff process.
