Job Purpose: Physical therapists contracted or employed through the organization are responsible for providing physical therapy services and for adherence to all conditions outlined in the personnel service agreement.
Responsibilities- Provides physical therapy services to patients in accordance with the physician’s written plan of care. Services include assessment, treatment planning, and implementation of therapeutic interventions.
- Assesses and evaluates patients’ functional status, including muscle strength, mobility, gait, range of motion, rehabilitation potential, and home environment, and determines physical therapy priorities.
- Develops, initiates, and directs individualized physical therapy treatment programs to meet patient goals.
- Instructs and educates patients, families, caregivers, and involved personnel on therapeutic exercises, treatment goals, safe techniques, and the use and care of therapeutic and prosthetic devices.
- Monitors patient response to treatment and reports changes in condition or progress to the appropriate personnel.
- Identifies patient and family/caregiver needs for additional home health services and makes referrals as appropriate.
- Prepares and submits required clinical documentation, including evaluations, progress notes, and clinical summaries based on patient outcomes and attainment of goals, in accordance with organizational policy.
- Participates in interdisciplinary case conferences and discharge planning.
- Provides consultation and education to families, caregivers, and nursing personnel as indicated.
- Participates in peer consultation and quality improvement activities.
- Supervises physical therapy assistants and, when applicable, home health aides in accordance with organizational policy and state regulations, including required supervisory visits and evaluations.
- Performs other job‑related duties as assigned.
