Private Duty Caregiver

Avenues Home Care, formerly Sentinel Homecare, seeks compassionate caregivers to provide private duty care to the elderly, disabled, and vulnerable clients in the local area, including Brentwood, Nashville, Murfreesboro, and more.

• Assist clients with Activities of Daily Living (ADLs) as outlined in their personalized care plans: bathing, dressing, grooming, toileting, transferring, eating, continence care, etc.
• Provide medication assistance, housekeeping, laundry, meal preparation, feeding, shopping, transportation to appointments, and companionship.
• Complete daily activity reports through the agency management system and maintain accurate logs.
• Communicate regularly with supervisors via phone, email, agency app, or in‑person.

• Practice safety and maintain client confidentiality.
• Be able to lift or push up to 50 pounds.
• Maintain punctuality and adhere to scheduling requirements.
• Report suspected abuse/neglect and document any unusual events.
• Attend meetings and in‐services as required.
• Maintain HIPAA‑compliant confidentiality of records.
• Other duties as assigned.

• Strong verbal and written communication skills.
• Ability to follow verbal and written assignments.
• Proficiency using the Homecare Management System—Clear Care.
• Effective interpersonal, organizational, and planning skills.
• Maintain “Fit for Duty” physical condition, valid driver’s license, automobile insurance.

• Age 18 or older.
• CNA license or equivalent caregiver experience preferred.
• CPR & First Aid certifications preferred.
• Reliable transportation.

• Prior experience assisting with ADLs or caring for elderly, disabled, or vulnerable clients.

• Telephone, smartphone, or tablet.
• Personal Protective Equipment.

• Follow safety rules and work safely.
• Report unsafe working conditions and behaviors.
