This role will assist in the growth of our business and deliver high-quality patient care.

• Build and maintain relationships with local providers and referral sources.
• Lead community outreach to increase client census.
• Provide clinical leadership for the care team.
• Conduct initial assessments and weekly care management.
• Triage clinical situations.
• Coordinate care with the Interdisciplinary Team (IDT) and create care plans for caregivers.
• Provide clinical support to in‑home caregivers.
• Deliver nursing education to patients, family and staff as needed.
• Provide basic bedside nursing care.
• Perform intake assessments and consultations.
• Engage in community outreach/support.
• Manage medication.
• Coordinate care and patient advocacy.
• Transport clients to appointments and outings.

• Educational Minimum Requirement: an Associate Degree in Nursing;
  Preferred.
• Candidate:
  Bachelor’s or Master’s Degree in Nursing.
• Preferred Med/Surg, Emergency or Intensive Care Experience.
• Must be a Registered Nurse (RN) in the state for which they are applying.
• A minimum of two (2) years’ experience in geriatrics, home care, home health, and/or care management required; other professional experience may substitute.
• Ability to work in a team environment.
• Ability to interface effectively and professionally with clients' family, physicians, attorneys, and other professionals included in client’s case.
• Excellent written and verbal communication skills required.
• Technical literacy required.
• Valid driver’s license and proof of current auto insurance required.
