The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The centers provide campus-based direct services and supports at 13 centers located throughout the state.

Responsibilities- Perform moderately complex social services work and provide technical assistance to assist interdisciplinary team (IDT) members, SSLC leadership, residents and families/guardians/LARs to access and navigate programs and services related to the SSLC.
- Serve as liaison to guardians, family members and LARs for routine or non-routine notifications; assist in preparing and coordinating with social workers and admissions staff; gather, review and document information; prepare assigned reports.
- Provide technical assistance and administrative support to the transition specialist to coordinate community referrals; develop training materials for group education; conduct individual and group trainings; monitor compliance with policies.
- Assist in rights and decision-making assessments, trust fund requests, and training on topics including admissions, transfers, end-of-life issues and guardianship.
- Organize events, marketing and coordination for SSLC programs and activities; support disaster response, COOP and other agency requirements as assigned.
- Attend regular work and shifts as required; may include on-call, overtime or rotating schedules.
- Serve as liaison to guardians/LARs for notifications, address concerns, and coordinate with families and IDT to address issues.
- Participate in meetings, provide information at residence/family meetings, and deliver training on assigned topics.
- Review case information for new admissions, prepare social histories, and gather information on residents’ support systems and needs.
- Coordinate community referrals with transition staff; assist in obtaining documents (, birth certificate, SSN) and prepare administrative memos for placements.
- Provide rights-based training and support and monitor policy compliance; conduct investigations and revise policies as needed.
- Assist with residents’ trust funds and financial processes; coordinate with families and staff for purchases as needed.
- Support admissions, transfers, transitions, discharges and furloughs; assist families and IDT with end-of-life issues and guardianship timelines.
- Encourage program awareness; organize events and provide facility tours and community speaking engagements as assigned.
- Conduct surveys and stakeholder feedback processes; prepare reports and take action to improve service quality.
- Perform other duties including disaster response/COOP as needed.
Skills And Abilities
(KSAs)
- Knowledge of Texas Administrative Code, HHSC policies, and the Department of Justice Settlement Agreement; understanding of client protections and human rights for ICF-IID.
- Ability to market programs, identify performance measures, and use computers (Microsoft Word, Outlook, PowerPoint, Excel).
- Skill in facilitating meetings and group/individual training; ability to navigate programs and services; gather and analyze facts; collaborate on solutions; communicate effectively with staff, residents and families.
