General Summary of Duties

The Care Coordinator functions as a member of a multidisciplinary team, planning, coordinating, and providing treatment and support services to program clients. Provides education, consultation to families, and crisis intervention services to clients assigned to the team.

Responsibilities
  • Provide care coordination services for an assigned group of clients, including individual, group, family counseling, crisis intervention, and case management.
  • Facilitate child and family team meetings and support additional meetings as needed.
  • Create and update the Plan of Care for each family.
  • Build community partners and identify resources to support the family’s unique needs.
  • Complete CANS screening, reassess every three months, and document client progress.
  • Develop, write, implement, evaluate, and revise overall treatment goals and plans.
  • Educate and support clients’ families and advocate for clients’ rights and preferences.
  • Assist clients in finding a primary care physician, dentist, and coordinate psychiatric care with medical care.
  • Provide crisis intervention counseling and coordinate with crisis services when appropriate.
  • Document client progress and maintain a permanent record.
  • Participate in daily staff meetings and treatment planning review meetings.
  • Coordinate substance use treatment with internal and external providers.
  • Assist clients with gaining resources and education to support their autonomy and basic needs.
  • Monitor LRA compliance and facilitate extensions with court personnel.
  • Perform any other responsibilities appropriate to the position.
Minimum Qualifications

Education: Bachelor’s Degree in psychology, social services, or behavioral health field.

Experience: Entry level; no prior related work experience.

Driving Requirements: Must meet agency driving requirements, minimum age 21, motor vehicle history check, valid Washington state driver’s license, reliable, insured transportation.

Licensure: Agency Affiliated Counselor Registration.

Preferred Qualifications
  • Minimum 2 years related experience.
  • Experience in co‑occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment.
  • Capacity to work well with children and families.
Performance Requirements Knowledge
  • Working knowledge of current trends in community mental health and health care reform.
  • Certification as a Child and Adolescent Needs and Strengths Assessor and administer screening.
Skills
  • Establish and maintain effective working relationships.
  • Clear and concise communication.
  • Computer proficiency with PCs, fax, printer, copier.
Abilities
  • Meet or exceed productivity standards.
  • Collaborate as a team member.
  • Build positive working relationships with all agency departments.
Equipment Operated

Standard office equipment including computers, fax machines, copiers, printers, telephones.

Work Environment

Frequent mobility and/or sitting required for extended periods.
