We are seeking the right candidate for the position of CNA Household Coordinator at Brandon Community Care Center. In this vital role, you will ensure that our residents receive the highest level of care while collaborating with a dedicated team and providing exceptional clinical leadership and support.

• Lead and mentor the household, overseeing the social function and coordinating activities.
• Partner with the Clinical Coordinator to coordinate care for residents within the household.
• Develop positive resident relationships, explore aging approaches, and support families.
• Address and document special psychological concerns (death in family, health decline, etc.).
• Maintain individualized psychosocial assessment and evaluate personal and emotional concerns.
• Assist with completing an individualized activity assessment.
• Maintain household activity calendar and coordinate activities, ensuring resident appointments are kept.
