We are currently seeking an ambitious Clinical Rehabilitation Specialist to join our team on a PRN basis. A Clinical Rehab Specialist acts as a coordinator between internal and external referral sources and acute inpatient rehab operations.

• Establishing relationships with case managers, physicians, payers and other referral sources.
• Ensuring appropriate patient placement through clinical screenings and assessments in collaboration with the hospital case management team and the Rehab Medical Director.
• Working closely with the hospital case management team to ensure a responsive and smooth-flowing referral and admission process for the inpatient acute rehab program.

• Graduate of an accredited school in a clinical specialty.
• Minimum 2 years of recent clinical care experience.
• Licensed professional in a clinical health care specialty (RN, Physical Therapist, Occupational Therapist, Speech Therapist, etc.).
• Pass Functional Independence Measure (FIM) within 90 days upon hire and maintain current certification.
• If RN, CRRN preferred and/or achieved within 2 years of hire.
• BCLS certification (AHA).
• Clinical experience managing inpatient rehabilitation patients.
• Experience as a clinical liaison coordinating admissions for inpatient rehabilitation patients.
