Job description
Reporting to the Director, Cardiovascular, Chronic Disease and Stroke, the Nurse Practitioner (NP) will be responsible for the provision of optimal care within the Cardiac program.
Accountabilities:
Responsible and accountable for the comprehensive assessment of patients and clients including diagnosis diseases, disorders and conditions for both Heart Failure outpatients and admitted inpatients with various cardiac diagnosesInitiates treatment including health management, therapeutic interventions and prescribes medications in accordance with the statutory and regulatory standards, limits and conditions, and employer policies and proceduresProvide professional guidance to other healthcare professionals and participates autonomously as a member of the interdisciplinary health teamInitiate referrals to specialist physicians, as appropriateCollaborate with clients, health care professionals and cardiologists to identify and assess trends and patterns that have implications for clients, families and communitiesDevelop and implement population and evidence-based strategies to improve health, and participate in policy making activities that influence health services and practicesParticipates in peer review and self-review to evaluate the outcome of the services at the client, community and population levelResponsible for personal professional development, and maintaining competence in the positionParticipates/initiates research where an opportunity arisesJob qualifications
Current registration, and in good standing, with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (proof of registration required)Membership in professional organizations (NPAO)Master’s degree requiredCompletion of Coronary Care Nursing courses requiredMinimum two-year, current experience in the NP role caring for cardiac patients at all stages of the cardiac continuum of careCore competencies:
Current BCLS requiredACLS preferredCompletion of the certificate in “Hospital & Acute Care for Nurse Practitioners” preferred or willing to obtain within 18 months of hireKnowledge of computerized data management and statistical analysisAbilities to perform the full scope of duties and responsibilitiesAbility to apply knowledge of pharmacology, pharmacokinetics and pharmacodynamics to prescribing and evaluating treatment responsesAbility to communicate and collaborate with patients and families about health findings, diagnosis, treatment, self-care and prognosisAbility to collaborate, consult with and formally refer clients and patients to physicians and other health professionals when appropriateAbility to critically assess and evaluate health research literature to determine best practices; ability to introduce education and evidence-based researchAbility to assess and recognize population health trends; ability to plan and implement strategies for population-based prevention and health promotionAbility to implement and evaluate planned changesAbility to define the specific areas of practice and client population for whom the position is competent to provide health care servicesAbility to supervise others and evaluate care they deliverAbility to lead a team and work within a teamAbility to self-direct, interact and adapt effectively with other professionals in complex, dynamic situationsAbility to transfer knowledge, coach and mentor othersAbility to identify and respond appropriately to legal and ethical issues that may arise in patient careAbility to self-assess performance and assume responsibilities and accountability for own professional development, educational or consultative…
