Senior Staff Nurse - Unplanned Care

Are you a passionate and caring nurse with at least two years' community experience, looking to develop your clinical and leadership skills within a dynamic community nursing team? If so, we would like to meet you.

A vacancy has arisen within the Unplanned Care Team, which delivers innovative nursing care through rapid responses to patients in their homes. The role involves assessing, planning, managing and evaluating presenting issues, helping to prevent avoidable hospital admissions, and supporting early discharges. You will work closely with the clinical band‑7 to provide daily leadership and contribute to the effective management of the team, while liaising with the wider multidisciplinary team.

We are seeking a highly motivated individual who is ready to meet the challenges of community nursing and provide high-quality care in patients' homes. As part of the Unplanned Care Team, you will work alongside a range of professionals to achieve the best outcomes for patients.

The role operates on a rotational basis as part of a 24/7 service, predominantly nights with day rotation, supported by agile technology to deliver timely, safe and effective care.

The successful candidate will demonstrate excellent clinical skills, strong leadership qualities, and a commitment to delivering high standards of patient care and experience.

Main duties of the job

Senior staff nurses will have a minimum of 12 months' community nursing experience. The postholder will assess, plan, implement, monitor, and review patient care.

The senior staff nurse role supports the district nurse team leader with the daily service coordination and delivery; and is pivotal in the communication within the team.

To work as part of the community nursing team in unplanned care, ensuring high quality health care is delivered to patients in their own home.

The post holder will be responsible for their own workload and work in collaboration with the wider Community Partnerships and Primary Care Networks.

The post holder will be responsible for the assessment of health needs, delivery of care to the practice population, prevention of hospital admissions and facilitation of early discharges from hospital.

The unplanned service is delivered over a seven‑day rota, covering a 24‑hour shift pattern. Working agilely across the service.
