Job Overview
Bradford City Primary Care Network 4 is looking to recruit a forward-thinking and enthusiastic experienced Pharmacist to join our existing Pharmacy team.
Key Responsibilities- Provide proactive leadership on medicines and prescribing systems to the PCN multidisciplinary team, patients and their carers
- Develop and deliver patient-facing services and clinics which improve patient outcomes, reflect the needs of the PCN population and shift workload from other disciplines
- Deliver direct patient consultations and prescribing as part of the practice and cluster multidisciplinary team approach for acute illness
- Improve patient and carer understanding compliance with their medicines
- Provide pharmaceutical consultations and home visits to patients with long term conditions
- Field medicines and prescribing queries from patients and staff
- Organise and oversee the PCN medicines optimisation systems including the repeat prescribing and medication review systems
- Deliver high quality medication reviews and develop bespoke medicines treatment plans for patients (particularly the elderly, complex patients and those at high risk of admission)
- Improve the safety and quality of prescribing following hospital admissions and attendance
- Improve the quality and effectiveness of prescribing through clinical audit and education to improve performance against NICE standards, prescribing guidance and other clinical standards
- Develop yourself and the role through participation in clinical supervision, training and service redesign activities
- Contribute to patient safety culture through reporting, investigation of medicines incidents and proactive measures
- Provide a clear and active link with community pharmacists and other stakeholders in the medicines supply chain to improve patient experience and manage issues
- Deliver training, mentoring and guidance to other clinicians and staff on medicines issues
- Contribute to the achievement and maintenance of good to outstanding CQC registration status>
- Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
- Mandatory registration with the General Pharmaceutical Council
- Holds or working towards an independent prescribing qualification
- Membership Primary Care Pharmacy Association (PCPA)
- Membership of the Royal Pharmaceutical Society
- Clinical Diploma or equivalent (demonstrate experience)
- Minimum of two years experience as a pharmacist, demonstrated within a practice portfolio
- Experience and an awareness of the breadth of common acute and long-term conditions that are likely to be seen in general practice
- Demonstrate ability to integrate general practice with community and hospital pharmacy teams and community groups
- Experience of partnership/collaborative working and of building relationships across a variety of organisations
- Experience of working in primary care
- Experience of working in a GP practice
- Minimum of 2 years post qualifying experience
- Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities
- An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
- Knowledge of IT systems, including ability to use word processing skills, emails and the internet to create simple plans and reports
- Able to plan, manage, monitor and review general medicine optimisation issues in core areas for long term conditions
- Able to gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers
- In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
Hours: Full Time (37.5hrs) or Part Time
Annual Leave Entitlement: 33 days including bank holidays (pro rata for part-time)
Increased holiday entitlement in recognition of length of service
