An exciting opportunity has arisen to work as a Breathe Co-ordinator (Smoking Cessation) to support our patients struggling to quitsmoking. This co-role works in system partnership between Healthy Lifestyle Solutions CIC, BD4+ Primary Care Network, BD4+ Community Partnership and Bradford Council Living Well service. The post holder will work with patientsbelonging to Bowling Highfield Medical Practice, Low Moor Medical Practice,Rooley Lane Medical Practice and Tong Medical Practice.

  • The Breathe Co-ordinator will work from clinical records and identify people who would benefit from quitting smoking. They will thenoffer very brief interventions over a period of time to support them either toaccess the Living Well Swap to Stop Smoking offer or accessing a Swap to Stop3-month free vape. Along the way it is envisaged that they will need othersocial support which can be provided by the social prescribing team or bysignposting to relevant services.
  • To support smokers by using a trauma informed approach and recognising smoking isnt a lifestyle choice and support people in a similarwayto those with long term conditions. The postholder will utilise differentapproaches to behaviour change such as motivational interviewing, coaching and Making Every Contact Count (MECC) to support patients to start their quitattempt.
  • The Breathe Co-ordinator alongside other key services provided by Healthy Lifestyle Solution CIC and the wider BD4+ Team will establish group interventions targeted at specific groups e.g., pregnant peoplewho smoke, and older people with COPD.
  • The Breathe Co-ordinator will also support the developmentpeer-led groups and help people draw on their lived experience to accesscommunity resources to enable each other to improve their lives and makehealth-related behaviour changes.
  • People will be reviewed regularly (group review) and willhave the option to graduate from the service, with the promise that they canreturn to the group (through a process mirroring patient-initiated follow-up) should they feel they need to access support again to keep on track.
  • Provide personalised support to individuals, their families, and carers to take control of their health and wellbeing, live independently and improve their health.
  • Develop trusting relationships by giving people time andfocus on what matters to me.
  • Provide very brief advice at events within the communityincluding schools, places of worships, community centres/hubs and targetedhealth events. They will also signpost people where to get support.
  • Take referrals from a wide range of agencies, including

    PCNs GP practices, MAST, and multi-disciplinary team such as pharmacies,hospital discharge teams, allied health professionals, fire service, police,job centres, social care services, housing associations, and voluntary,community and social enterprise (VCSE) organisations (list not exhaustive) andact on these in a timely manner.
  • Work closely with the Public Health Living Well Serviceto support their Tobacco Control agenda.
  • Work closely within the PCN with key clinicians as wellas non-clinical team members.
  • Represent BD4+ Community Partnership and the Core
    20plus5health intervention which is tackling health inequalities out in the community and at associated meetings.
  • Post holder will be proactive in developing strongpartnership links with all local agencies to encourage referrals, recognisingthe need to be confident in the service to make appropriate referrals.
  • The post holder will be working in a developingenvironment, and he/she will therefore be expected to undertake otherappropriate duties as required.
  • Maintain accurate written records and monitoring an devaluation of the service examples include Swap to Stop and Core 20+5monitoring.
  • Understanding of the wider determinants of health, includingsocial, economic, and environmental factors and their impact on communitiesindividuals, their families, and carers. Understanding of, and commitment to,equality, diversity, and inclusion. Knowledge of IT systems, including abilityto use word processing skills, emails, and the internet to create simple plansand reports.
