Overview

District Nurse Caseload Manager - Planned Care - City & South Locality

Responsibilities
  • Lead and develop a dedicated team of community nurses and healthcare support workers, ensuring safe, high-quality and compassionate care in people s homes and community settings.
  • Provide advanced assessment, care planning and case management, working in partnership with patients, families, carers, GPs, and wider multi-disciplinary teams.
  • Support prevention of hospital admissions, facilitate timely discharges, and enable people to remain independent for as long as possible.
  • Ensure team members are appropriately trained, supported and have the right knowledge, skills and competencies to meet role requirements.
  • Balance clinical and non-clinical duties (approximately 80% clinical and 20% non-clinical) and manage the day-to-day running of the caseload with clinical responsibility.
Qualifications / Experience
  • Qualified District Nurse with leadership abilities and experience in community nursing and planned care.
  • Strong assessment, care planning, and case management skills, with ability to mentor and develop staff.
  • Ability to work in partnership with patients, families, carers, GPs and wider multidisciplinary teams.
Requirements
  • Right to work in the UK; sponsorship eligibility as required for the role.
  • Registration with the appropriate regulatory body and compliance with professional standards.
  • Desirable: knowledge of safeguarding, discharge planning and community health care governance.
