• Conduct physical therapy assessments and develop individualized treatment plans based on clinical findings and physician recommendations
• Implement evidence-based interventions to improve mobility, strength, balance, and functional independence
• Monitor, evaluate, and document patient progress, adjusting care plans as needed
• Educate patients and families on treatment plans, home exercise programs, and safety strategies
• Collaborate with healthcare professionals and provide in-home therapy services to support patient recovery and quality of life

• Licensed Physical Therapist in the state of Florida
• At least 1 year of experience in physical therapy
• At least 1 year of experience working in a Home Health environment
• Reliable transportation and a valid driver’s license
• Passionate, professional, and committed to excellent care

• Job Types: Full-Time | Part-Time | Per Diem | Contract
