Position Type: Full-Time
Location: Home health visits in Manatee County (Bradenton, Lakewood Ranch, Ellenton, Palmetto, Bradenton Beach, Anna Maria Island, and surrounding areas)
Schedule: M-F 40 hours with weekend/holiday on-call rotations
Responsibilities- Perform comprehensive patient assessments, including OASIS assessments at required intervals
- Develop, initiate, and revise individualized plans of care as patient needs evolve
- Deliver skilled occupational therapy services and treatments as outlined in the plan of care
- Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
- Collaborate with the interdisciplinary team to ensure seamless, holistic care
- Active Florida PT license
- Minimum of 1 year experience as a licensed Physical Therapist (PT)
- Home health care experience strongly preferred
- Preferred experience using Homecare Homebase (HCHB) EMR
- Strong assessment, critical thinking, and patient education skills
- Willingness to participate in weekend and holiday on-call rotations
