Bay Care Home Care is looking for a Home Health Aide Per Visit to provide excellent customer service and personal care to patients in the Bradenton area. The role is based at Bay Care Home Care and involves visiting patients in Manatee County, FL.

• Work under the guidance and supervision of a staff nurse to provide personal care to patients.
• Follow the patient’s plan of care to complete tasks assigned by the registered nurse or other skilled professional.
• Maintain open communication with the patient, representatives (if any), caregivers, and family members.
• Demonstrate competency with assigned tasks.
• Comply with infection prevention and control policies and procedures.
• Report changes in patient condition and honor patient rights.

• CNA (Certified Nursing Assistant) or HHA certification from an accredited 75‑hour program.
• Florida driver’s license required within 30 days of hire.
• BLS (Basic Life Support).

• High School Diploma or equivalent (preferred).

• Preferred 1 year in home care (other experience may be considered).
