Learning Response Lead – Champion patient safety and drive change!
This is an exciting role for a dynamic and resourceful individual who has a genuine interest and experience in patient safety and who is willing to further develop in this specialist field.
It is a senior role to support our team with meeting the requirements of the Patient Safety Incident Response Framework (PSIRF) specially focusing on leading a high quality process for our learning responses including Swarm Huddles, After Action Reviews, MDT Round tables and Thematic Reviews.
The Learning Response Lead will work as part of a wider multi-disciplinary team across the organisation to ensure the delivery of robust patient safety learning responses; to investigate, review and analyse, using recognised tools encompassing principles of human factors and ergonomics, systems engineering, psychology, and recognised best practice.
The postholder will work 37.5hrs per week, ideally 7.5 hours a day (exclusive of breaks) over five days. This is currently a Monday to Friday job and will work predominantly from home using virtual technology, although there is a requirement to be able to travel across the organisation boundaries, maintain regular face to face contact with colleagues and attend face to face meetings as required.
Our current head quarters’ base is in Bracknell (London House).
- Lead high-quality learning responses to patient safety incidents, driving meaningful improvements in care across our Trust.
- Responsible for coordinating and facilitating comprehensive patient safety reviews (PSRs), applying human factors and systems thinking alongside compassionate engagement with patients, families, and staff, ensuring sensitive and effective learning processes.
- Collaborate with clinical, corporate, and executive stakeholders, embedding a just culture and endorsing psychological safety.
- Deliver training on our key learning response methodologies supporting local teams to understand their application.
- Present findings at key internal forums and influence change at every level.
- Support and inspire a culture of continuous learning and improvement in physical health, mental health, and children’s service.
- Communicate effectively and empathetically with families and front-line staff directly involved in an incident.
- Produce excellent report writing, manage conflicting priorities, and complete patient safety reviews to a high standard.
- Manage highly sensitive and potentially distressing information.
