RN - Case Management

This role operates on a 3x12s schedule, with night shifts from 7pm to 7:30am, and may include some weekend requirements.

The Resident Care Coordinator is responsible for the coordination, development, implementation, and evaluation of resident care and clinical practice to ensure a high standard of care is delivered. This role serves as a clinical resource and leader for the care team, providing guidance, supervision, and intervention in the delivery of resident care and the care planning process in accordance with all regulations and policies.

• Coordinates, supervises, and evaluates the care of residents in skilled nursing, assisted living, or similar facilities.
• Schedules and trains care staff, ensuring that clinical services are delivered in accordance with regulations and professional standards.
• Communicates with families and medical professionals as needed, ensuring care plans for residents are properly documented and updated.
• Provides care and compassion to foster residents' physical and emotional well-being.
• Participates in performance improvement activities to enhance the safety and care of residents.

• American Heart Association BLS certification.
