* Required:
** - Montana Licensure (Registered Nurse) - American Heart Association BLS Certification - Minimum of 2 years of nursing experience - MUST HAVE UTILIZATION REVIEW EXPERIENCE
** Preferred:** - Bachelor’s Degree in Nursing - 2 years of acute care hospital experience as a case manager

* Essential Job Functions:
** - Complete comprehensive assessments of members per regulated timelines.
- Develop and implement case management plans in collaboration with members, caregivers, physicians, and support networks.
- Perform ongoing monitoring of care plans, documenting interventions and goal achievement.
- Maintain an ongoing member caseload for regular outreach and management.
- Promote the integration of services for members, enhancing continuity of care.
** Knowledge, Skills, and Abilities:** - Demonstrate sound judgment and professionalism at all times.
- Thrive in a busy and stressful environment.
- Exhibit strong interpersonal, verbal, and written communication skills.
- Maintain confidentiality and exercise tact and discretion.
- Proficient in computer applications, including MS Office and EMR.
- Possess strong organizational and prioritization skills.
