• Deliver exceptional care to the dedicated client in their home.
• Help execute comprehensive personalized weekly schedules and care plans.
• Provide skilled nursing services as prescribed under a client's plan of care that can be safely provided in the home, within your scope of practice.
• Counsel clients and families in meeting nursing and related needs.
• Meet all documentation requirements according to agency policy and procedure.
• Coordinate services by participating in case conferences, case management, team meetings, and staff meetings as indicated.
• Perform administrative functions such as documentation of completion of tasks and reporting changes in the client’s physical condition, mental capability or behavior.

• Licensed Practical Nurse (LPN) license required.
• BLS required.
• Experience placing peripheral IVs, accessing ports and working with PICC lines required.
• Ability to use sound judgment and remain calm in a variety of situations.
• Basic computer skills.
• Proof of eligibility to work in the U.S.
