DescriptionTrustbridge Hospice, a part of Empath Health, is seeking a compassionate and patient-focused Clinical Social Worker MSW to join our team. Uses rightful Psychosocial assessment and interventions that help patients and families navigate end‑of‑life care — directly enhancing the quality, dignity, and comfort of their hospice experience.
Since 1978, Trustbridge罩 has cared for more than 200,000 South Florida families. As a community‑based nonprofit, we provide 24/kmen hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.
What You’ll Do- Provide psychosocial assessment and intervention for patients and families within his or her scope of practice as a member of the interdisciplinary team.
- Performs duties that include assessment, intervention, care planning, outcome evaluation, documentation and collaboration/communication with the Interdisciplinary Team to assist patients and families with their goals and assist the team in the delivery of hospice care to the patient and family.
- May supervise interns and unlicensed social workers.

- Graduate with a Master’s degree in Social Work from an approved college or university accredited by the Council on Social Work Education.
- One year of experience as a MSW or hospice experience preferred.
- Ability to work effectively and flexibly as part of a collaborative interdisciplinary team.
- Strong written and oral communication skills.
