The Physical Therapist is responsible for providing medically prescribed, goal-oriented therapy in accordance with the Physician Plan of Care
, company policies, and professional standards of practice. This role also involves coordinating care delivery and collaborating with the interdisciplinary team to ensure the highest standards of patient outcomes.

• Develop and implement individualized patient physical therapy plans of care.
• Provide in-home visits, evaluating and reassessing patient needs.
• Communicate with physicians and care team members as needed.
• Deliver education and instruction to patients, families, and team members.
• Participate in interdisciplinary team meetings to review and coordinate care.
• Ensure care plans are regularly reviewed, updated, and documented accurately.
• Adhere to all agency policies, Federal/State regulations, and ACHC accreditation standards.
• Complete electronic documentation timely and accurately using the EMR system.

• Current Physical Therapist (PT) license in the state of Florida and in good standing.
• Specialized training/experience in pelvic floor therapy

• Knowledge of physical therapy principles, concepts, and methodologies.
• Excellent written, verbal, and interpersonal communication skills.
• Strong problem-solving and decision-making abilities.
• Ability to work effectively with an interdisciplinary healthcare team.

• Current CPR certification
• Level 2 Background Screening
• Recent negative TB / CXR
