Position Summary:
This position provides direct care services to assigned clients with mental illness and adjustment difficulties and accesses services for clients. These services are provided through individual, group and family therapy, through the diagnostic assessment process or through individual community based rehabilitative services and/or group rehabilitative services including linkage and advocacy as necessary. Services are provided in accordance with governing laws, regulations and certification requirements and must be identified on the Individual Treatment Plan (ITP).
Key Responsibilities & Role Highlights:
- Provides diagnostic assessment, when applicable, treatment planning, counseling services and psychiatric supportive treatment services to an assigned caseload of clients in accordance with state mandates, established agency and program policies, procedures, and philosophy.
- Demonstrates the knowledge and skills to provide appropriate care to clients with mental illness including understanding their clinical needs, symptoms, and behaviors as well as the principles of growth and development over the life span.
- Assess data about each client’s status and interpret the information needed to identify everyone’s requirements in relation to his or her age‑specific needs and provide care that is appropriate.
- Maintains customer service procedures according to agency guidelines.
- Uses current DSM effectively in providing diagnosis, when interacting with clients and planning for treatment.
- Completes diagnostic assessments and diagnostic updates as applicable within agency standards.
- Establishes and implements a written Individualized Treatment Plan (ITP) within established time frames based on program and individual goals and objectives to an assigned caseload. The Individualized Treatment Plan is measurable and specific. Coordinates services and providers, monitors progress of goals/ objectives and need for clinical review of the assigned client’s mental health assessments.
- Provides individual therapy as needed.
- Provides restorative interventions and training to improve interpersonal, community integration and independent living skills when the mental illness impacts the client’s ability to function in and adapt to home, school, work and community settings.
- Makes referrals, both internally and externally, according to the ITP and monitors outcome.
- Monitors and records symptoms of decompensation. Reports symptoms to client's provider.
- Coordinates with other service providers and follows through with client needs.
- Obtains and serves as health officer, as needed.
Education & Experience Requirements:
- Requires a bachelor’s degree in social work or a master’s in counseling. Master’s in social work preferred.
- Must be at least 21 years of age.
- Must demonstrate knowledge of mental illness, evidence‑based practices, motivational and crisis intervention skills.
- Must demonstrate knowledge in chronic physical health issues and needs of people who are SMI.
- Must demonstrate knowledge of care planning and care coordination in integrated health.
- Must demonstrate knowledge of evidence based and developmentally appropriate screening.
