Position: Care Manager / Social Work Assistant - 27 hours per week

The pay range for this role will be £31,537 - £36,363 (pro rata for part time colleagues).

An exciting opportunity has arisen to join the Community Rehabilitation Team as a Care Manager within Adult Social Care. This role is offered on a part‑time basis, with a commitment of 27 hours per week. As a Care Manager, you will collaborate closely with health and social care partners to support individuals living with significant mental health needs within the community.

Your work will focus on enabling recovery, fostering independence, and facilitating safe discharge planning where appropriate.

Main Responsibilities
  • Conduct assessments and reviews in line with the Care Act and Section 117, ensuring that all processes adhere to statutory guidance and best practice.
  • Develop strengths‑based plans and manage risk in partnership with multi‑disciplinary colleagues, ensuring that planning is person‑centred and collaborative.
  • Coordinate support services that promote independence and enable timely transitions, including supporting individuals through the hospital discharge process where relevant.
  • Maintain safeguarding practices and ensure that all records are kept to a high standard, reflecting accurate and up-to-date information.
The Ideal Candidate
  • An enthusiastic individual with relevant experience in adult mental health or community rehabilitation settings.
  • Confidence in working within multi‑agency partnerships and the ability to communicate in a clear and professional manner.
  • Excellent communication, organisational, and digital record‑keeping skills.
  • A working knowledge of the Care Act, Mental Capacity Act, and strengths‑based practice.
  • To hold a valid driving licence and have access to a car.
