We’re looking for an experienced Family Nurse Practitioner who loves working with patients, values clinical autonomy, and wants to practice medicine without the friction and compromises of insurance-based care.

Cloud Medical is an independent Direct Primary Care (DPC) practice offering proactive, longevity-focused primary care through a membership model. Free from traditional insurance constraints, we provide comprehensive, relationship-based care supported by advanced diagnostics, personalized care planning, and long-term clinician–patient partnerships.

Ownership & Responsibility:

• See approximately 13–15 patients per clinical day through scheduled visits in-office and telehealth
• Provide comprehensive primary, urgent, and preventive care
• Build long‑term relationships with patients focused on proactive health optimization
• Perform office‑based procedures and women’s health including Pap smears, injections, and related services
• Support preventive screenings and routine in‑clinic care
• Deliver functionally informed, data‑driven care using advanced diagnostics, labs, and wearable medical devices
• Interpret results and guide patients through personalized care plans
• Respond to patient messages and clinical questions through the patient portal in a timely, professional manner
• Oversee and collaborate with Medical Assistants during triage, procedures, and follow‑up care
• Partner with the care team to optimize workflows and deliver a seamless patient experience
• Participate in case reviews, team huddles, and clinical meetings
• Provide scheduled on‑call clinical coverage as part of the provider rotation

• Active Family Nurse Practitioner (FNP) license in good standing
• Experience delivering primary care to adult patients
• Strong clinical judgment and comfort with independent decision‑making
• Excellent written and verbal communication skills
• Willingness to work on‑site at our Longmont & Boulder clinics

• Experience in Direct Primary Care, concierge, or cash‑based models
• Interest or experience in longevity, functional, or preventive medicine
• Comfort using advanced diagnostics and health data to guide care
• Experience in innovative or fast‑growing clinical environments

Schedule:

8am - 5pm Mon - Thurs; 8am - 2pm Friday
