We are a physician owned physiatry practice, dedicated to improving the quality of life and therapeutic outcomes for patients in skilled nursing and long-term care facilities. By collaborating closely with the primary care teams in these facilities, we increase patients' ability to participate in physical therapy while reducing the need for pain medications.

We are a group of skilled providers (physicians, PAs, and NPs) offering care across ten states, with plans for expansion to all 50 states. For the past four years, we’ve partnered with top‑tier nursing and skilled care facilities nationwide to manage pain and enhance therapy outcomes, helping thousands of patients regain independence and comfort.

We are seeking a passionate, motivated provider to join us full‑time and independently offer care and consultations at our contracted facilities.

As a physiatrist, you will round at 1–3 Skilled Nursing Facilities or similar rehabilitation centers, 1–3 days per week at each location. Your primary focus will be delivering specialized physiatry care to 25+ patients per day who are experiencing difficulty participating in therapy due to pain or functional limitations.

• Perform focused assessments and develop individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
• Provide targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient‑specific needs.
• Serve as the sole physiatrist on‑site, operating with a high level of autonomy while supported by a collaborative clinical team for consultation as needed.
• Coordinate care and refer to other specialties or services for issues beyond the scope of physiatry.

• Current, unrestricted PA or NP license.
• DEA licensed or willing to obtain DEA licensure.
• Peri nursing home and/or physiatry experience is a plus.
• Comfort and interest in performing peripheral joint injections (knees, shoulders, hips).
• Strong communication skills and collaboration ability.
• Familiarity with EHR charting.

• Hands‑on training in facility settings to ensure consistent, high‑quality care.
• Flexible 5‑day workweek with optional weekends, no nights or on‑call shifts.
• Competitive production‑based salary (starting at $130,000+ per year).
