Join Signify Health as a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations, connecting patients to appropriate care and improving health outcomes.
Base pay range$41.00/hr – $90.00/hr
Job highlightsThe typical visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry). You’ll use an iPad and an intuitive clinical workflow. You are not required to prescribe medicine, order lab tests, or alter treatment regimens.

- Work with Signify Health as a clinician colleague, fulfilling the specified role requirements
- Be flexible to travel locally and within licensed states, as needed, and conduct virtual visits as required
- Provide all Signify Health services, including diagnostic and preventive services such as spirometry, DEE, bone density screening, etc.
As an in‑home clinician, your primary workplace is the private residence of health plan members. You will work in diverse environments that may include household pets, secondhand smoke, and other potential allergens or irritants. You must be able to perform your duties safely in these settings.
Per Diem Employees Qualify For- Hourly base rate plus incentive structure (e.g., per visit completed)
- Malpractice insurance coverage during Signify Health–related activities
- Supplies and other perks
Preferred Qualifications (NPs and PAs)
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi‑state licenses (preferred)
- Availability to cover primary locations:
Sterling and surrounding area

Pay RangeThe typical pay range for this role is $41.85–$90.13 per hour, plus additional reimbursement for completed diagnostic and preventive services.
