In-Home Health – Nurse Practitioner or Physician Assistant (Part Time) – Pueblo, CO

Join Signify Health to provide in‑home health evaluations focused on personalized, preventive care for patients in their own homes.

Base Pay Range

$41.00/hr – $90.00/hr

In This Role, You Will

• Conduct in‑home evaluations lasting about 45 minutes, including medication and medical history review, physical examination, and diagnostic testing (e.g., diabetic eye exams, spirometry).
• Use an iPad and intuitive clinical workflow to facilitate visits.
• Travel locally and within licensed states as required, and conduct virtual visits when needed.
• Provide Signify Health services such as diagnostic and preventive services (spirometry, DEE, bone density screening, etc.).

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Multi‑state licenses (preferred)
• Ability to cover the following primary locations:
  Fremont, Saguache, Custer, Alamosa, Costilla, Huerfano, Pueblo, Crowley, Otero, Las Animas

$41.85 – $90.13 (base hourly rate or base annual full‑time salary, depending on experience, education, geography, etc.)
