In-Home Health – Nurse Practitioner or Physician Assistant (Part Time) – Pueblo, CO
Join Signify Health to provide in‑home health evaluations focused on personalized, preventive care for patients in their own homes.
Base Pay Range$41.00/hr – $90.00/hr
In This Role, You Will- Conduct in‑home evaluations lasting about 45 minutes, including medication and medical history review, physical examination, and diagnostic testing (e.g., diabetic eye exams, spirometry).
- Use an iPad and intuitive clinical workflow to facilitate visits.
- Travel locally and within licensed states as required, and conduct virtual visits when needed.
- Provide Signify Health services such as diagnostic and preventive services (spirometry, DEE, bone density screening, etc.).

Fremont, Saguache, Custer, Alamosa, Costilla, Huerfano, Pueblo, Crowley, Otero, Las Animas
