This is a full-time position with a client caseload in Boston MA, Brighton MA, Brookline MA, and surrounding areas.

Responsibilities
  • Ensure all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the Home Health team leadership.
  • Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
  • Assess assigned caseload of home health care patients and families to identify physical, psychosocial, and environmental needs, as evidenced by documentation, clinical records, Interdisciplinary Team reports, after‑hours reports, and on‑site evaluations.
  • Assume primary responsibility for caseload, including assessment, planning, coordinating, implementing, and evaluating the plan of treatment.
  • Assess patient needs and obtain data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care.
  • Make the initial evaluation and re‑evaluate the patient’s nursing needs during each visit. Make follow‑up visits according to patient, family, and facility needs.
  • Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other personnel involved with patient care.
  • Revise the plan of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility, and other team members.
Qualifications
  • Current, unrestricted state RN license.
  • 1+ year experience in a clinical care setting as a nurse.
  • Associate’s Degree or Bachelor’s Degree in Nursing is preferred.
  • Previous Home Health or Hospice Experience is preferred.
  • Previous experience with Home Care Home Base is preferred.
  • Able and willing to travel within the branch/office coverage area; must be able and willing to travel 50%.
  • Ability to sit, stand, bend, lift and move intermittently and be able to lift 50‑100 lbs.
  • Valid driver’s license and insurance and reliable transportation to perform job tasks.

You will report to the Clinical Manager or Clinical Supervisor.
