Description

As a Registered Nurse (RN) at Fenway Health you will provide compassionate, evidence-based, and efficient care to individuals, families, communities, and patient populations. The Registered Nurse's care delivery is consistent with the Massachusetts Board of Registration in Nursing, the ANA Scope and Standards of Practice, and the ANA Code of Ethics. At Fenway Health the RN will provide quality nursing care to patients consistent with the care philosophy of patient-centeredness, non-judgmental, harm reduction, behavioral health integration, and team-based care. The nurse serves in a clinical leadership role to the care team with primary responsibilities for disease management, care coordination, and advice/triage needs for the care team's panel of patients. The nurse ensures that the team functions smoothly and efficiently while keeping the patients' well-being paramount.

The schedule is based on clinic needs and may include covering evening and Saturday clinics on a rotating basis.

Nursing is the protection, promotion, and optimization of health and abilities, prevention of illness and injury, facilitation of healing, alleviation of suffering through the diagnosis and treatment of human response, and advocacy in the care of individuals, families, communities, and populations.

Principal Duties and Responsibilities

* Collaborates with the patient's provider, specialist providers, and other members of the care team to improve patient outcomes through a patient-centered care plan incorporating patient engagement and education to promote health and prevent injury, illness, and disease exacerbation.

* Provides competent, compassionate nursing care based on the nursing process, clinical reasoning, objective scientific principles, and the patient's subjective experiences.

* Conducts virtual, telephonic, or in-person nurse assessment and triage to assess the severity of the patient's health concerns using approved protocols and resources to advise appropriately.

* Implements the patient-centered plan of care, evaluates outcomes, and regularly communicates the plan of care, patient status, and progress toward goals with the provider, patient, and other care team members.
