- The Center for Specialized Services (CSS) is a dynamic, diverse, and highly customer-centric department designed to attract international, executive, and concierge patients to Mass General.
- Responsible for assuring competent, compassionate, individualized, nursing care for patients and loved ones, including delegation to and supervision of non-professional clinical and support staff.
- Build excellent rapport with patients, their loved ones, and departments within the hospital.
- Collaborate with all members of the care team to ensure that patient care is coordinated, comprehensive, and that care transitions are seamless.
- Actively engages with the care team and practice leadership to identify opportunities and solutions to improve quality, patient access and cost savings.
- Provide evidence-based nursing care utilizing the nursing process and interdisciplinary teamwork and collaboration.
- Utilize assessment, critical reasoning and clinical judgment to expedite appropriate care and treatment.
- Integrate patient history and chronic disease knowledge to provide longitudinal care to a panel of patients.
- Document nursing assessment, interventions, plan and outcomes in the patient record in a timely manner.
- Create a therapeutic relationship with patient and family.
- Apply telecommunications technologies to assess, triage, provide nursing consultation, and perform follow-up and surveillance of patient status and outcomes.
- Provide health education to facilitate the patient treatment plan.
- Empower patient and family to manage their health care and navigate the health care system.
- Respect and support patient and family cultural beliefs, values, and treatment preferences; coordinate care with the health care team.
- Review and follow up with patients and health care team to ensure tests and referrals are current; evaluate efficiency of care and interventions for assigned group of patients.
- Coordinate continuity of care with inpatient and community teams.
- Demonstrate leadership to ensure safe, efficient and cost-effective care; identify opportunities and solutions for the practice environment.
- Participate in quality and safety initiatives and compliance with regulatory requirements; generate ideas to improve care delivery and workflow.
- Precept and mentor new team members; participate in departmental and organizational committees and initiatives.
- Interpret hospital and departmental policies and procedures to nurses and other team members; maintain and update clinical knowledge and skills.
