Overview
The Oncology Nurse Navigator (ONN) uses oncology-specific clinical knowledge to offer individualized guidance to patients, families and caregivers to foster independence during the cancer journey. Using the nursing process, an ONN assesses patient needs and provides education and resources to facilitate appropriate and timely quality health and psychosocial care throughout all phases of the cancer continuum. The ONN creates a patient-centered and compassionate care environment.

Responsibilities- Participates in day-to-day operations of the program including patient education and call triage
- Participates in program evaluation and development of strategic goals consistent with DFCI s mission and professional values
- Participates in QA/QI projects for the program
- Supports the research activities of the program
- Demonstrates professional commitment through integrity, honesty and accountability
- Delivers expert clinical care through evidence-based practice partnerships with patients, families and colleagues
- Assumes nursing care responsibilities for a defined group of patients within the program, including assessment, education, coordination and documentation
- Facilitates individualized care considering functional status, cultural factors, health literacy, psychosocial, reproductive/fertility, and spiritual needs
- Supports smooth transitions from active treatment into survivorship, chronic cancer management, or end-of-life care
- Assists patients with cancer regarding treatment goals, advanced directives, palliative care, and end-of-life concerns using an ethical, nonjudgmental framework
- Acts as a liaison between patients, families, caregivers and providers to optimize outcomes
- Advocates for patient-centered care including shared decision making and patient goals of care
- Provides psychosocial support and facilitates referrals for patients, families and caregivers
- Contributes to ONN program development, implementation, and evaluation within the healthcare system and community
- Tracks and monitors metrics and outcomes in collaboration with administration to document program outcomes
- Functions as the primary nursing contact for care delivered within the physician practice setting
- Engages in collaborative decision making using data and collective expertise
- Manages patient care across the continuum, including new patient calls and appropriate referrals
- Works collaboratively as an effective member of the health care team
- Monitors the environment of care with attention to patient safety and compliance with regulatory standards
- Maintains clinical practice that is evidence-based and aligned with nursing policies, procedures and standards
- Provides care based on age-specific needs of the patient
- Adheres to the Code of Ethics for Nurses with Interpretive Statements (ANA, 2001) in all professional practice aspects
- Complies with accepted standards of professional nursing practice and licensure/registration requirements applicable to the nursing role
- Fosters an environment sensitive to diverse populations including culture, ethnicity, gender and age
- Professional Development: identifies areas for growth; formulates goals and methods; serves on Nursing and Institute committees; supports research activities
- Meets annual evaluation requirements and competencies; collaborates on professional reports and presentations
- Pursues active membership in local and national professional organizations; commits to continuous learning and improvement
- Demonstrated ability to work effectively as a member of an interdisciplinary team
- Demonstrated critical thinking, communication, organizational, problem-solving skills, and ability to make independent decisions
- Licensed as a Registered Nurse in the Commonwealth of Massachusetts
- Baccalaureate degree in nursing required within 4 years of hire
- Minimum of 1 year (or equivalent) of oncology nursing experience
- Certification in Oncology Nursing (OCN, AOCN, or CPON) preferred
