Oncology Nurse Navigator - Cutaneous Oncology

The Oncology Nurse Navigator (ONN) utilizes oncology‑specific clinical knowledge to provide individualized guidance to patients, families, and caregivers, fostering independence throughout the cancer journey. Using the nursing process, the ONN assesses patient needs and delivers education and resources to facilitate timely, quality health and psychosocial care across all cancer continuum phases.
Responsibilities- Participates in day‑to‑day operations of the program including patient education and call triage.
- Engages in program evaluation and development of strategic goals consistent with DFCI’s mission and professional values.
- Participates in QA/QI projects for the program.
- Supports the research activities of the program.
- Exhibits professional commitment through individual integrity, honesty and accountability.
- Delivers expert clinical care through evidence‑based practice partnerships with patients, families and colleagues.
- Assumes nursing care responsibilities for a defined group of patients, including assessment, education, coordination, and documentation.
- Facilitates individualized care within the context of functional status, cultural considerations, health literacy, psychosocial, reproductive/fertility, and spiritual needs.
- Supports smooth transition of patients from active treatment into survivorship, chronic cancer management, or end‑of‑life care.
- Assists patients with cancer regarding treatment goals, advanced directives, palliative care, and end‑of‑life concerns using a non‑judgmental, nondiscriminatory ethical framework.
- Acts as liaison between patients, families, caregivers and providers to optimize outcomes.
- Advocates for patient‑centered care, shared decision‑making and patient goals of care.
- Provides psychosocial support and appropriate referrals.
- Contributes to ONN program and role development, implementation, and evaluation.
- Participates in tracking and monitoring metrics and outcomes with administration to document program results.
- Functions as primary nursing contact for care delivered within the physician practice setting.
- Engages in collaborative decision‑making utilizing data expertise and collective wisdom.
- Manages patient care across the continuum, including new patient calls and referrals.
- Works collaboratively as a member of the health‑care team.
- Monitors the environment of care with attention to patient safety and compliance with regulatory standards.
- Maintains evidence‑based clinical practice consistent with nursing policies, procedures, and standards.
- Provides care based on age‑specific needs.
- Adheres to the Code of Ethics for Nurses with Interpretive Statements (ANA, 2001) in all aspects of practice.
- Complies with accepted standards of professional nursing practice defined by DFCI and licensure requirements.
- Fosters a sensitive environment for diverse populations, including culture, ethnicity, gender, and age.
- Professional Development:

- Effective member of interdisciplinary team.
- Critical thinking, communication, organizational skills, problem solving, and independent decision‑making.
- Licensed as a Registered Nurse in the Commonwealth of Massachusetts.
- Baccalaureate degree required; bachelor’s in nursing preferred.
- Minimum of 1 year of oncology nursing experience.
- Certification in Oncology Nursing (OCN, AOCN, or CPON) preferred.
SUPERVISORY RESPONSIBILITIES: No
