Pay range: $41.71–$105.65 per hour
Duties & Responsibilities
- Coordinate and insure implementation of the plan of care using case‑management principles.
- Within 24‑48 hours of admission interview patients/families, consult physician team to develop provisional treatment program and tentative discharge date.
- Review daily treatment plan with physicians, nurses, and families to ensure interdisciplinary communication.
- Participate in patient care rounds to contribute to plan of care and monitor progress.
- Collaborate with other departments to expedite sequencing and scheduling of interventions, consults, treatments, and ancillary services.
- Ensure daily continuity with patients, addressing discharge‑related needs.
- Incorporate utilization‑management principles and payer contracts into patient plans.
- Present alternatives to inpatient stay to attending MD, team, and patient/family based on assessed level of care and insurance benefits.
- Seek assistance from Care Coordination leadership for outlier or resource‑intensive patients.
- Interact with internal and external providers for post‑discharge services.
- Contribute to development, implementation, and monitoring of practice guidelines.
- Identify learning needs related to case management and develop education plans.
- Execute discharge planning: assess continuing care needs, coordinate family meetings, ensure patient education, identify service options, promote interdisciplinary communication, perform follow‑up, and initiate contact with home health agencies.
- Collaborate with payers to ensure admission appropriateness, discharges, and reimbursement; conduct utilization reviews; identify and address payer concerns; provide reports on utilization trends.
- Graduate of an approved school of nursing with current Massachusetts registration;
Bachelor of Science in Nursing required for newly licensed nurses and external candidates. - Minimum 1 year acute‑care experience; 1 year inpatient case‑management experience required.
- Strong clinical assessment, interpersonal, and organizational skills; ability to work collaboratively with diverse teams.
- Bilingual (English/Spanish) preferred.
- Excellent written and verbal communication; data compilation from medical records; negotiation of care coordination aspects.
- Ability to work independently in a busy, sometimes stressful hospital environment.
