Healthcare Nursing, Nurse Practitioner, Clinical Nurse Specialist
Responsibilities
- Deliver preventative, follow‑up, and urgent care to patients of the practice, collaborating with MD partners, primary nurses, medical assistants, and support staff to deliver patient‑centered team‑based care.
- Independently manage a panel of patients of their own and be responsible for meeting quality and panel targets as outlined by the department.
- Co‑manage patients within the practice with designated physicians.
- Perform and record the patient history and physical exam; form an assessment and plan, order appropriate testing and imaging, interpret results, and communicate findings to patients. Consult with and seek physician collaboration, including specialists when appropriate.
- Complete documentation, act upon labs, imaging, and other studies promptly and in accordance with institutional and departmental guidelines and policy.
- Respond to patient questions and messages in accordance with institutional and departmental guidelines and policy.
- Perform medically delegated aspects of patient care as agreed upon by nursing and medical administrators according to practice guidelines.
- Maintain and practice in accordance with practitioner guidelines per state and hospital regulations.
- Maintain and update clinical knowledge and skills based on current health‑care practice/research.
- Maintain the standards and educational requirements specific to the position and licensure.
- Complete patient notes and associated documentation within the requisite time frame in accordance with institutional and departmental guidelines and policy.
- Evaluate the outcomes of patient care using patient satisfaction information, MWI, ED & readmission rates, and other data as made available by Primary Care Administration, MGPO and other appropriate sources within the institution.
- Participate in and reinforce all practice‑based process improvement efforts.
- Serve as a resource to nurses, medical assistants, and staff for clinical questions/problems and participate in care when indicated.
- Participate in precepting student Nurse Practitioners in accordance with institutional and departmental guidelines and policy.
- Promote cooperation and support collegial relationships among all staff.
5–7 years clinical experience preferred as a Nurse Practitioner in primary care. Master's degree in nursing required. Current Board certification as an Adult or Family Nurse Practitioner required.
Skills and Abilities- Extensive clinical experience in primary care, urgent care, and chronic disease management.
- Experience managing patients independently, and co‑managing patients with physicians.
- Outstanding time‑management and organizational skills.
- Demonstrated ability to effectively interact with physicians and coworkers.
- Strong written and verbal communication skills.
- Experience with common PC computer software including Microsoft Word, PowerPoint, Excel, Access, and Outlook; experience using clinical data capture systems such as EPIC.
