Renal, Pulmonary and GI, with overflow of other medical/surgical services. Common complex medical case types with or without complex co-morbidities.

Job Profile Summary

This role focuses on providing professional and nonprofessional nursing care services in accordance with physician orders. It includes performing clinical nurse duties: assesses, plans, implements, and evaluates the care needs of a designated group of patients or provides nursing care and identified clinical support tasks within the scope of practice to achieve the patient’s plan of care. Requires a nursing license.

This is a professional individual contributor role that may direct the work of other lower-level professionals or manage processes and programs. The role involves overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education. This senior level role may manage projects/processes, coach and review work, and resolve difficult problems.

Overview

The professional registered nurse possesses the skills and knowledge necessary for delivery and management of patient care from admission through discharge. The nurse stays informed on healthcare trends, technology, pharmacology, treatment modalities, quality and patient safety. The nurse serves as patient and family advocate while fostering a collaborative environment among health care providers. They plan, deliver and manage patient/family-centered care using the nursing process and standards of nursing practice embedded in research, evidence-based practice and/or best practices.

The nurse provides clinical excellence to support a patient- and family-centered model of care.

Job Description

Minimum Qualifications:
  • Registered Nurse (RN) license.
  • Basic Life Support (BLS) certification.
  • Certifications (e.g., ACLS, PALS) relevant to the department and type of care may be required.
  • Preferred Qualifications:
  • Bachelor of Science in Nursing (BSN).
  • Specialty nursing certifications.
  • Duties and Responsibilities
  • Utilize theoretical and evidence-based knowledge to partner with patients and families to assess, plan, implement and evaluate age-appropriate care.
  • Assess and collaborate with patient, family/significant others and other disciplines to identify expected outcomes and develop a care plan that meets cultural needs.
  • Plan individualized outcomes-based care plans, orchestrate and validate the plan of care for each shift, and coordinate discharge planning with resources to ensure seamless transitions.
  • Implement the plan safely and timely; document modifications; collaborate with nursing colleagues; use evidence-based interventions; provide health teaching; document care per policy and regulatory requirements.
  • Evaluate the effectiveness of planned strategies, update the care plan based on patient response and outcomes.
  • Demonstrate critical thinking and apply evidence-based practice; provide culturally sensitive care; initiate changes in nursing practices as appropriate.
  • Perform procedures and treatments according to unit guidelines and professional standards.
  • Safely administer medications and assess patient response; ensure pharmacology knowledge aligns with standards of care.
  • Maintain clinical competency with respect to technology and equipment.
  • Act as an advocate and educator for patients and families.
  • Provide high-quality, efficient, and safe care; use data trends to support practice improvements.
  • Participate in safety and quality initiatives and performance improvement activities; support patient safety initiatives.
