• Assess and Adapt: Each visit starts with evaluating your patient s current condition, medical needs, and overall well-being. No two patients are the same, and no two days look alike.
• Create and Modify Care Plans: You re actively shaping your patients  treatment plans based on what they need, adjusting care as conditions change.
• Teach and Support: Whether it s a psychiatric patient needing medication guidance or a post-op patient learning about wound care, you play a vital role in helping them stay independent at home.
• Communicate and Coordinate: If something changes, you re the one who keeps providers, families, and care teams informed, ensuring patients get the right care at the right time.
• Document and Reflect: At the end of your day, you submit your notes from home, ensuring every visit is recorded accurately for compliance and for the next nurse who continues care.

• Active LPN license in Massachusetts.
• One year of nursing experience (home health or psych preferred).
• A drive to provide compassionate, patient-centered care.
• Reliable transportation for patient visits.

Position is per diem in the Greater Boston area.
