Position: Activities Coordinator (Bilingual Haitian Creole)

MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

Monday–Friday | Day Shift | Full-Time (40 Hours)

Summary

The Activities Coordinator works directly with individuals with developmental disabilities, leading both group and one‑on‑one recreational activities tailored to their needs, capabilities, and interests.

Essential Job Functions
  • Conduct group and individual recreational activities based on participants’ needs, abilities, and interests.
  • Implement necessary precautions to ensure the care and safety of individuals served.
  • Collaborate with the interdisciplinary team to design meaningful group activities that emphasize participant preferences.
  • Provide instruction and guidance to staff on activity delivery methods and techniques, both daily and during staff meetings.
  • Maintain and update curriculum binders for each classroom to ensure varied, age‑appropriate activities are consistently offered.
  • Participate in continuing education courses as required.
  • Perform other related duties as assigned.
Qualifications
  • High school diploma or equivalent; must be 18 or older.
  • Two years of experience supporting individuals with intellectual and developmental disabilities, preferably older adults.
  • CPR and First Aid certifications required.
  • Strong attention to detail with effective communication skills.
  • Reliable, responsible, and compassionate work ethic.
  • Bilingual in English and Haitian Creole required.
