Job Summary

About Brookside Community Health Center:
Brookside is a licensed Community Health Center of Brigham and Women s Hospital and provides multi-disciplinary, comprehensive care with services in a range of medical areas including Adult, Family and Pediatric Primary Care, OB/GYN, Family Planning, Dental/Oral Surgery, Nutrition/WIC, Behavioral Health and Specialty Medicine (Cardiology, Pulmonology and Nephrology). Brookside also has phlebotomy, financial counseling, and community resource specialists onsite. Each year, more than 12,000 patients from Jamaica Plain, the city of Boston and surrounding areas use these services, producing more than 69,000 visits to Brookside each year.

Most of Brookside’s patient population are Spanish-speaking and socioeconomically under-resourced. The health center s services, guided by equity, are provided by a bilingual English/Spanish medical and professional staff. Brookside is a teaching site for Harvard Medical School, Mass General Institute of Health Professionals School of Nursing, Harvard Dental School, Boston University Dental School, as well as other Boston area health care professional programs.

Summary

The Social Worker is responsible for performing social work assessments and interventions as needed for inpatients, outpatients, and emergency department patients. They are dedicated to patient and family-centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate, and ethical problem-solving. As a member of a multidisciplinary health team, they perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations.

Social Workers are responsible for working collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.

Essential Functions
  • Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
  • Formulates biopsychosocial assessment, disposition, and treatment plans.
  • Collaborate with other professionals to evaluate patients  medical or physical condition and to assess client needs as part of the interdisciplinary team.
  • Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
Qualifications

Education
:
Master of Social Work Degree (MSW) or Master of Counseling/Psychology/Related field from an accredited program required.

Licensure
:
Current Massachusetts Licensed Independent Clinical Social Worker (LICSW) or current Massachusetts Licensed Mental Health Counselor (LMHC).

Language
:
Spanish strongly preferred.

Experience
:
Clinical experience in a medical setting required (2-3 years).
