Job Summary

The Patient Care Assistant II functions as a member of the patient care team, under the direction of the Registered Nurse (RN), and proficiently and consistently performs all aspects of the PCA role. The Patient Care Assistant II is responsible for supporting the RN with clinical tasks, including but not limited to blood glucose monitoring, completing EKGs, applying simple dressings, using aseptic technique and responding to medical emergency situations utilizing Heartsaver CPR and AED skills.

The PCA II is also responsible for responding to the needs of patients and family members and taking an active role in the efficient operation of the unit. The PCA II will also assure the practice of the Patient and Family Centered Care Model in an environment that exemplifies best practice.

Responsibilities
  • PROVISION OF A SAFE ENVIRONMENT:
    Anticipates and identifies patient safety risks, cooperates with licensed nurses, and takes action to reduce risks. Examples include maintaining bed height, using side rails and alarms appropriately, placing call lights within patient reach, performing frequent patient checks, and assisting with ambulation as indicated.
  • Makes routine patient room rounds to address patient needs.
  • Follows and maintains restraint policies under the supervision of a licensed nurse; prior to utilizing any restraints, consults with the licensed nurse.
  • Provides 1:1 monitoring as a patient observer for patients at risk for injury.
  • Follows established hospital infection control and safety procedures.
  • Uses proper body mechanics when moving or assisting patients to prevent personal injury.

  • ORGANIZATIONAL ETHICS:

  • MANAGEMENT OF RESOURCES:
    Organizes time and prioritizes patient care; assists co-workers; adapts to changing unit needs; coordinates break periods with the Charge Nurse/Nursing Supervisor; maintains visibility on the unit.
  • MANAGEMENT OF INFORMATION:
    Reviews policies and procedures; understands emergency codes; uses hospital email for communications; attends staff meetings and in-services; maintains verbal and written communication with RN; documents patient data accurately and timely; notifies RN of significant changes in patient condition or monitoring alarms.
  • COLLABORATIVE INTERDISCIPLINARY:
    Maintains a teamwork approach to patient care.
Patient/Family Education
  • Provides information to patients and families, orients to the hospital/unit/services, distributes patient mail/flowers, and ensures necessary items are accessible (including call lights, nourishments, linens, clothing).
  • Reinforces patient education provided by the nurse and offers basic instructions (e.g., call light use, positioning, transfers, walking, toileting, feeding).
Competency
  • Complies with standards, policies, procedures, and regulations (e.g., DPH, JCAHO, Boston fire codes, OSHA).
  • Maintains unit competencies and demonstrates skills such as external monitor use, CPM, EKGs, glucose monitoring, dressings, first aid/CPR/AED.
  • Mentors new PCTs and serves as a resource to float staff.
Quality Improvement
  • Participates in SEAQ programs; communicates quality concerns to leadership.
Patient Care
  • Performs patient care tasks as assigned by the RN, per policy and procedure, including:
  • Assists with admission, discharge, transfer, room preparation, and patient identification.
  • Transports patients and belongings; provides personal care (bathing, grooming, hygiene, linen changes); performs basic skin care; applies dressings; assists with elimination; supports comfort measures and rest; provides 1:1 monitoring as needed; performs simple dressing changes; assists with treatments; prepares meals and assists with feeding; records intake/output and vital signs; performs EKGs and glucose monitoring; documents effectively; provides end of life care (comfort measures only and post mortem care);
