Overview
The Team Clinician promotes the full integration and functioning of adults with psychiatric conditions by providing person centered clinical community services. Supporting the Adult Clinical Community Services (ACCS) model, the Team Clinician will work in a community-based setting and perform clinical duties including intake, assessment, treatment planning and clinical interventions whether the client lives independently in the community, in a Group Living Environment or a Supported Independent Environment.
Responsibilities- Determine needs for rehabilitative services as required.
- Provide leadership amongst the team by directing the interventions provided by Team Outreach Workers.
- Oversee treatment interventions in GLEs/SIEs and model appropriate interventions.
- Train and support staff in delivering clinical evidence-based practices.
- Initiate and conduct required intake and engagement processes with Lead Peer Specialist for assigned referred Persons Served within 72 hours to begin building the therapeutic relationship and assess service needs.
- Engage and assess treatment and support needs of Persons Served, initially and in a sustained manner.
- Determine needs for rehabilitative services as required by Rehab Option for GLE/SIE and outreach PS. Oversee information collection for assessments and develop Medicaid Rehab Option billable treatment plans in collaboration with the PS, Peer Specialist(s) and other staff.
- Complete, date and sign Mental Status Exam and Clinical Formulation-Interpretive Summary; sign and date Treatment Plan and subsequent Reviews/ Revisions.
- Conduct Treatment Plan meetings with PS and involved parties; discuss discharge criteria and plan for discharge in subsequent meetings.
- Lead intervention team for each PS, including Outreach Worker(s), Nurse, Housing Coordinator, Addictions Recovery Counselor, Peer Specialist, Community Connector, Employment Coordinator, and/or Family Connector based on PS needs.
- Oversee service delivery to all assigned PS, including participation in services, recovery goal planning, readiness for discharge, and support through transitions including housing changes.
- Develop and deliver individualized, life stage appropriate services and supports to PS, including young adults and seniors.
- Collaborate with Team members to deliver recovery, community, addictions, health, family or other treatment or support services for Treatment Plan success, whether living independently or in a GLE/SIE.
- Promote healthcare and employment services access for PS.
- Promote engagement, safe housing, community tenure, self-sufficiency and successful completion of service for all PS.
- Assign and direct Outreach Workers to deliver skills training and supports needed for Treatment Plan success for PS in independent settings.
- Train and direct program staff to deliver interventions needed for Treatment Plan success in GLEs/SIEs.
- Perform, train, supervise, mentor and model evidence-based clinical and recovery-oriented interventions (e.g., Motivational Interviewing; Harm Reduction; SBIRT; Housing First; Stages of Change; WRAP; Seeking Safety; CBT).
- Ensure seamless integration of Team and GLE/SIE staff efforts in interventions for each PS Treatment Plan.
- Ensure thorough documentation of PS needs and services by Intervention Teams.
- Support competency-based training for staff to achieve successful outcomes; promote trauma-informed, recovery-oriented, person-centred, culturally competent approaches.
- Provide outreach services to PS as needed when disengagement is suspected.
- Oversee and assist PS during care transitions and life stage transitions.
- Collaborate with Assistant Team Leader to obtain and maintain guardian ships for PS lacking capacity to consent to treatment.
- Develop Safety Plans and Crisis Prevention plans with PS; review and update as factors change or at annual reviews.
- Interface with Emergency Services as needed and with ACCS Employment and Care Coordination entities.
- Conduct face-to-face visits within 48 hours of PS return to the community after ED visit, arrest, incarceration, homelessness, or hospitalization.
- Visit each assigned GLE/SIE twice weekly (or more as needs dictate) to supervise staff and monitor progress.
- Provide regular field supervision to Outreach Workers and receive role-based clinical supervision from ACCS Assistant Team Leader.
- Provide on-call clinical coverage as scheduled and participate in the LPHA/Clinician Community of Practice Workgroup for professional development.
- May cover Assistant Team Leader duties during absences and participate in other assigned activities.
- Perform other related duties as assigned.
