Bay Cove pursues this mission by providing individualized and compassionate services to people facing the challenges associated with developmental disabilities, mental illness, substance use disorders, and homelessness at more than 175 program sites throughout Greater Boston and Southeastern Massachusetts.

Job Summary: As a member of the PACT multidisciplinary team you support this mission by carrying out rehabilitation and support functions, by assisting in treatment, substance abuse services, education support, consultation to families, and crisis intervention under the clinical supervision of PACT leadership. Additionally, as a team clinician you will be designated to take on a specialized role based on the needs of the team.

This is an exempt position.

Supervisory Responsibilities: As designated by the Team Leader

Schedule: Monday through Friday 8am-4pm

Essential Functions of Position:

  • Collaborate with the Team Leader to coordinate engagement, assessment, and treatment planning processes for all individuals admitted to the PACT team.
  • Collaborate with the Team Leader to ensure completion of the Critical Needs Assessment and the Critical Needs Plan at the time of admission.
  • Coordinate the completion of the Comprehensive Assessment for each Person Served by
  • Ensuring releases are obtained,
  • Assigning appropriate trained team members to complete each section,
  • Assisting assigned Individual Treatment Team (ITT) members with scheduling Persons Served/family member interviews,
  • Tracking he ongoing ACA process, and
  • Working with the assigned Primary Care Manager to complete the clinical formulation and to identify the prioritized assessed needs.
  • Coordinate the treatment planning process and timely completion of all treatment planning documentation by
  • Coordinating the scheduling of the treatment planning meeting,
  • Facilitating treatment planning meetings as needed,
  • Assisting in the identification of appropriate goals and objectives,
  • Coaching staff to complete IAPs and reviews as scheduled before the deadlines, and
  • Ensuring the communication of new interventions to the team and documentation of said interventions in the cardex.
  • Provide expertise to the team regarding therapeutic skills and interventions, as well as clinical documentation skills, in the form of individual and group training as needed.
  • Engage in individual, group, and family therapy according to assessed needs.
  • In the absence of the Team Leader, assume program leadership including facilitation of the Daily Organizational Meeting, support of the daily Shift Manager in problem solving when issues arise, coordinating communication with the PACT Psychiatrist when not on-site, and facilitation team clinical discussions and treatment planning processes.

Team Role Functions

  • Serve as Primary Care Manager for assigned Persons served by coordinating and monitoring the activities of the Individual Treatment Team (ITT), by assuming responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in conjunction with the Person Served and the ITT.
  • Conduct, or contribute to, the comprehensive assessment of Persons Served.
  • Provide treatment and rehabilitation services to all persons served by the team including skills training and support in all areas of functioning including: symptom management, coping skills, substance abuse, vocational and educational, housing, activities of daily living, benefits counseling, money management, public transportation, appointment attendance, social and recreational, etc.
  • Collaborate with natural supports and extended support networks as requested by Persons Served.
  • Participate in and contribute to daily organizational meetings and treatment planning meetings.
