Case Manager in Boston. Immediate Start!!
Responsibilities- Provide direct services to people in the areas of housing, risk mitigation, transportation, skill building, money management, medication administration and other treatment based plans.
- Work closely with families and early intervention provider staff, to ensure proper services and supports are delivered according to early intervention best practices.
- Collaborate with the patient and patient s family or significant others to implement an effective service plan, explaining the available clinical options to the team, including the advantages and disadvantages of each option.
- Communicate and document patient s progress toward their recovery.
- Associate s or bachelor s degree in a related field and/or equivalent to two years relevant human services experience with population that has persistent mental illness and/or substance use disorder.
- Bi-lingual fluency in Spanish is preferred not required; lived experiences are welcomed and valued
- Ability to complete and pass Criminal Background Check
- Establish a relationship with one of the nation s largest referral agencies for behavioral-health.
- This is a client temp-to-perm negotiable opportunity.
