Boston Medical Center (BMC) invites applications for the licensed social worker position within the Care Management Department. The role focuses on coordinating discharge planning activities for short‑ and long‑term placements and supports patients and families in managing psychosocial challenges.

Essential Responsibilities / Duties- Effectively coordinate discharge planning activity in cases requiring social work/Care Management team intervention.
- Act as case manager, mobilizing and monitoring all adjunct activities necessary to effect appropriate discharge plans, including securing appropriate financial resources, and supporting patients and families through clarification and communication of the discharge planning process.
- Develop discharge plans based upon functional assessment in collaboration with patient, family, physician and health care team.
- Implement discharge plans and arrange for appropriate post‑hospital care by referring patients to appropriate transfer facilities or home health/home care agencies.
- Provide consultation to physicians, health care team, and other professionals by providing essential psychosocial information diagnostic formulations, plans and treatment recommendations.
- Participate in team meetings and coordinate patient care meetings with in‑house and outside agencies.
- Support department policies, procedures and practices for documentation and providing high quality care to ensure compliance with the guidelines of J.C.A.H.O, the Department of Public Health, and other regulatory agencies as required.
- Thoroughly review patient charts (as applicable) and always document services.
- Evaluate community resources for quality and appropriateness and develop and maintain referral and transfer relationships.
- Complete department statistical reports, agency specific referral forms, and other required documents from regulatory programs.
- Complete Medicare and other application forms as appropriate.
- Provide information/referral services.
- Provide consultation to Interdisciplinary Team.
- Participate on committees as assigned regarding patient care issues/service enhancement.
Education and Experience
- Master’s Degree in Social Work, Psychology, or Counseling and 0‑2 years of overall experience working with patients and families.
- Or equivalent combination of education and experience.
- Current licensed Clinical Social Worker in Massachusetts.
- LICSW.
Education and Experience
- 1‑2 years of discharge planning experience.
- Knowledge of local community resources preferred.
- Excellent computer skills, knowledge of Microsoft programs, data analysis.
$27.88 – $40.38 per hour.
