The mission of Charles River Community Health (CRCH) is to partner with individuals and families so they can thrive and lead healthier lives by delivering the comprehensive, integrated, and equitable primary healthcare that matters most to them. The Integrated Community Health Worker will serve as a frontline public health worker. The Integrated Community Health Worker serves as a liaison between health and social services and the community.
As such, the CHW will facilitate access to services and improve the quality and cultural competence of service delivery. If you have a calming presence, excel at understanding patient needs, and are willing to work on educating patients about the importance of health screenings, we would like to have you as part of our team. The CHW will work as an on-floor team member at our health center and will interact with patients and staff who are racially, ethnically, and culturally diverse, so cultural competence is required.
YOUR RESPONSIBILITIES
- Establish trusting relationships with patients and their families while providing general support and encouragement.
- Provide ongoing follow up, basic motivational interviewing and goal setting with patients/families.
- Serve as a liaison between clinical staff and patient, by following up with patients regarding their appointments, referrals, and needed resources.
- Help patients set personal goals and attend appointments.
- Provide referrals for services to community agencies as appropriate.
- Help patients connect with transportation resources.
- Help patients connect to needed resources to carry out plan of care (Meals on Wheels, home health, Prescription assistance, etc.).
- Work closely with integrated medical team to ensure that patients have comprehensive and coordinated care.
- Remain knowledgeable about community resources appropriate to needs of patient/family.
- Continuously validate and evaluate the effectiveness of the community resources.
- Provide consistent communication to Medical Provider/Nurse to evaluate patient/family status and report progress and compliance with identified plan of care.
- Enable and improve culturally and linguistically competent services and care by making efforts to understand the community culture and resources.
- Conduct outreach and implements programs in community that promote, maintain and improve community health needs and provides resources.
- Record timely and accurate documentation of patient engagements.
- Collect data to help identify community health needs.
- Advise community groups on chronic disease and diabetes self- management, hypertension, and nutrition.
- Advise clients on general health and exercise and self-care activities.
- Identify age-appropriate needs and resources.
- Follow up on hospitalizations and no shows.
- Provide situationally appropriate social support to the patient utilizing experience lived and learned through training provided at CRCH.
- Provide informal counseling as needed.
YOUR QUALIFICATIONS, COMPETENCIES, TRAITS
- Bachelor's Degree related field required.
- Bilingual speaking preferred in Portuguese or Spanish, but not required.
- BLS OR CPR Certification required, must be completed prior to start date, and must be hybrid or practical certificate including adult, pediatric and AED trainings.
- Community Health Worker Mass Board certification preferred, not required. Must obtain within 3-4 years of hire date.
- Preferred Medical Interpreter Certification.
- Must have knowledge of computer systems such as Word and Excel.
- Strong interpersonal, organizational, and time management skills required.
- Must have a willingness to work flexible hours to meet the organization's needs/demands.
- Must have excellent communication skills, particularly with people from diverse cultures, with the ability to understand the community, population, and patients we serve.
