Overview

Licensed Therapist for Geriatric Behavioral Health Services at Golden Plains Hospital. The role supports the hospital's mission by providing therapeutic services within the Lighthouse Intensive Outpatient Program and, when needed, to inpatients. Facilitates patient assessments and admissions to the Lighthouse Intensive Outpatient Program as assigned. Participates in Master Treatment Plans, Treatment Team meetings, and collaborates with interdisciplinary teams to develop discharge plans.

Maintains patient confidentiality and adheres to hospital policies and procedures.

• Facilitate patient assessments and admissions to the Lighthouse Intensive Outpatient Program as assigned.
• Provide therapeutic services to groups, individuals, and families in the Lighthouse program.
• Contribute to Bio-psychosocial assessments as assigned and update Master Treatment Plans accordingly.
• Participate in Treatment Team meetings and collaborate on discharge planning with an interdisciplinary team.
• Deliver therapy to individuals and/or families who are inpatients when needed.
• Maintain patient records and documents according to hospital policies and regulations; write reports and evaluations as required.
• Intervene in crisis situations when appropriate and support patient safety and wellbeing.
• Keep up-to-date with developments in counseling by reading relevant literature and attending courses or seminars.
• Represent Golden Plains Community Hospital and the Lighthouse program positively in all interactions.

• Licensed in the State of Texas as a Licensed Professional Counselor (LPC) or Licensed Marriage and Family Counselor (LMFT).
• Experience in providing therapeutic services in a geriatric behavioral health setting preferred.
• Experience providing group, individual, and family therapies.
• Ability to provide therapeutic services to ensure patient safety and program integrity; maintain a positive attitude at all times.

• Maintains patient confidentiality as outlined in HIPAA and program policies and guidelines.
• Has specialized knowledge of and empathy for the geriatric population.
• Interviews patients and completes patient assessments as needed.
• Completes bio-psychosocial assessments according to timelines in policies and procedures.
• Facilitates group therapy as well as individual and family therapies.
• Maintains patient records and documents according to hospital policies and regulations; writes reports and evaluations when needed.
• Guides patients in developing skills and strategies for addressing problems; teaches life skills to patients and families.
• Communicates effectively with referral sources, physicians, and families when appropriate.
• Is comfortable intervening in crisis situations to assist patients.
• Adds problems to Master Treatment Plan and monitors patients and evaluates treatment progress.
• Works with the interdisciplinary team to develop and implement effective discharge plans for each patient.
• Participates in community awareness activities as assigned; may lead classes or community events addressing health issues.
• Continually learns about new developments in counseling by reading professional literature and attending trainings.
• Represents Golden Plains Hospital and the Lighthouse program positively at all times.
